Madison - Isabella Grace Falkowski became an angel on April 23, 2012.
She is survived by her parents, Adam and Michelle; big sister, Lillian; grandparents, Michael and Denise West and Edward and Rosemary Falkowski and many uncles, aunts, cousins and great grandparents.
There will be a Graveside Service on Fri. April 27, 2012 at 10:30 a. m. at RESURRECTION CATHOLIC CEMETERY, 2705 Regent St. Madison, WI 53705 with Msgr. Kevin Holms presiding.
In lieu of flowers the family requests memorials to the Trisomy 18 Foundation.
Isabella will be forever loved and missed.
